Facilities Service Requests (FSRs)
Your request will be submitted to Facilites Management electronically by Tom. Once given a work order number, craftsmen will contact the person(s) listed on the FSR if necessary. The FSR is flexible - more work can be added to the same work order.

Estimates
Estimated jobs are for work services which are of a more complex nature, typically involve more than one craft, and go on for a longer period of time. On an estimated job, you receive back a fixed price for the work requested. The entire amount is liened when you approve the estimate: the job is then billed 75% at the time of the lien and the additional 25% when the job is completed.

Trouble Calls
For routine facilities issues such as room too hot or cold, light bulbs are out, dripping/running faucets or toilets, etc. When repoting the problem, it is important to include the room number.

Requesting Keys
Please fill out a key request form, have it signed by your Unit Head and submit to Tom Rosholt, 22480 YRL. If Library Building Services has the key in stock, the person requesting the key will be contacted. If the key must be ordered from the campus Hardware shop, an FSR will be prepared using the account listed on the request form.

Please note that keys must often be ordered through Facilites as we do not keep all copies in stock.

Desk and file cabinet keys do not require a key request form. Simply call Library Building Services to inquire if we have the particular key in stock or if an FSR is necessary.

If you do not have a Staff Intranet UserName and Password, or have forgotten your Password, send email to crt@library.ucla.edu from your Library email account. (Or Law or CLICC.)
